Appointments
Our office hours are 8:00 a.m. to 4:30 p.m. Monday through Friday. You may call during this time to schedule and appointment. If you are unable to keep your scheduled appointment, please give a 24-hour cancellation notice.

Insurance/Self-Pay
We participate in many insurance plans, including Medicare, Medicaid, and most HMO’s and PPO’s in the area. We also accept non-insured, self-pay patients. Our Business Manager is available to discuss the options available.

Fees, Billing, and Collection
All non-covered services, refractions, deductibles, and co-payments are due at the time of service. We are happy to discuss fees in advance and provide an estimate of our services. We accept, cash, check, MasterCard, Visa, Discover, and Care Card.

New Patient Information
Your initial comprehensive exam will likely include a thorough history taking, refraction, dilation, as well as a consultation with one of our physicians with the possibility of further diagnostic testing. For a complete eye exam, please allow two hours for your visit and we request that you arrive 10 minutes prior to your appointment. Please bring with you:

• Completed Patient Information Form and Patient History Form.
• Your current glasses, if applicable.
• A list of any medications and nutritional supplements you take regularly.
• Your medical and vision insurance cards.
• You might also consider bringing sunglasses or a friend or relative to drive you home as your eye will likely be dilated.
• Please remember that if the appointment is for a minor (under the age of 18), a parent or guardian must be present for the visit.
